FDA peptides advisory discussions have intensified as wellness-focused peptide products gain popularity while many remain unapproved and untested. Some peptides, such as the GLP-1 drugs Ozempic and Wegovy, have undergone full Food and Drug Administration review. Others are sold online in a largely unregulated gray market that researchers describe as multibillion-dollar in scale, with products lacking human-grade manufacturing and clinical evidence.

Recently, an FDA advisory committee recommended expanding access to several unapproved peptides by permitting their preparation and dispensing through compounding pharmacies with a prescription. These pharmacies create customized medications and operate under less oversight than traditional manufacturers, so they may distribute drugs that have not been FDA approved. Critics of the move argue the peptides have not been rigorously tested and could present risks.

The panel’s recommendation is not binding. The FDA would need to undertake formal rulemaking before any change takes effect. Opponents warn that broader availability could outpace scientific validation and potentially expose patients to uncertain harms.

What are peptides?

Peptides are short chains of amino acids, the building blocks of proteins. They are smaller and structurally simpler than proteins. In the body, many peptides act as signaling molecules, directing cellular functions. Scientists have identified about 7,000 endogenous peptides influencing processes from blood sugar regulation to mood. Examples include insulin, oxytocin, vasopressin and endorphins.

Insulin was the first peptide synthesized for therapy. More recently, GLP-1 agonists, which help signal satiety in the brain, have surged in use. While many therapeutic peptides are injected, some can be taken orally or via nasal spray. Any peptide intended to treat or prevent disease, or to affect the body’s structure or function, is legally considered a drug and subject to FDA oversight, though enforcement can vary. By contrast, nutrition-derived peptides such as collagen are supplements, which face looser regulation and greater variability in purity and potency.

Evidence-backed uses versus unproven claims

Peptides like insulin and GLP-1 agonists have extensive clinical data and FDA approval for defined conditions. Roughly 100 peptide-based therapies are approved for uses that include diabetes and multiple sclerosis. Alongside these are unapproved peptides marketed for general wellness or so-called biohacking. Many are sold online under a “research use only” label that functions as a legal workaround. Regulators have taken enforcement action against companies promoting such products explicitly for human use despite the label.

How U.S. rules may shift

In July, an FDA advisory panel reviewed seven unapproved peptides and voted to recommend increased access to six. These products are marketed for a range of uses, including muscle recovery, gut repair, inflammation reduction, metabolic support, cognitive enhancement and longevity.

The panel urged allowing compounding pharmacies to prepare and distribute the compounds, leaving the final decision to the FDA after rulemaking. According to Dr. Katrin Svensson, a peptide researcher and associate professor of pathology at Stanford University, this could reverse prior policy. She noted the FDA had restricted compounding of these and other peptides in 2023, deeming them too risky for pharmacies to make.

She emphasized that the peptides in question are not approved medicines, have not been tested in humans and have little to no preclinical data. Observers also point out that the advisory group includes several new members with ties to the peptide industry. Although the recommendation is nonbinding, greater permissiveness could encourage broader promotion of unregulated peptide use and potentially disincentivize rigorous research if products can be sold without definitive evidence.

What evidence underpins the recommendation?

Experts say no new scientific evidence drove the panel’s vote. Svensson said there is no fresh data supporting the shift. Long agreed, noting the decision does not establish that the peptides are safe, effective or scientifically validated, but rather could allow more people to obtain them through compounding pharmacies.

Will consumers benefit or face new risks?

Specialists say compounded peptides would likely be purer and sterile compared with research-grade versions sold online, which could reduce contamination risks. Long said that while compounding improves quality control, it still does not demonstrate safety or efficacy for human use or any specific condition.

Svensson cautioned that peptides encompass everything from established medicines to unproven experimental substances and that the shift is policy-driven, not science-driven. Easier access does not replace the need for controlled trials to determine dosing, benefits and risks.

What consumers should know

If the FDA permits compounding of the six peptides, products may be more consistently manufactured than uncontrolled online offerings. That would not confirm that they deliver on claims or are safe for long-term use, for combinations with other medicines, or for stacking with additional peptides. Svensson underscored that “allowed to be compounded” is not the same as “FDA approved,” and that for most wellness peptides, human evidence is limited, dosing is uncertain and vial contents can vary. Long said he would not self-inject experimental molecules.
